“We aim to fund community-led solutions that address health concerns with nonprofits in rural communities. This funding area allows us to support the specific health needs of organizations and address the social determinants of health. Funding is available to organizations outside the seven-county Twin Cities metropolitan area in Minnesota, in: North Dakota, South Dakota, and western Wisconsin; as well as any organization outside of the Omaha metro area in Nebraska, and southwestern Iowa. Rural Health grants will be available in Nebraska and southwestern Iowa by invitation only.”
