Under One Roof: Primary Care Models that Work for Adolescents

This report describes an interdisciplinary model of physical, behavioral and reproductive health care operating in a hospital-based adolescent medicine clinic, a private practice and a federally qualified community health center. Those three locations – Mount Sinai Adolescent Health Center (New York City), Wake Teen Medical Services (Raleigh, N.C.), and Erie Teen Health Center (Chicago) – meet the unique needs of adolescents, using multidisciplinary staffing, team-based approaches, staff sensitivity, teen-friendly environments and a focus on positive youth development.

The report also describes the funding challenges associated with providing integrated care and the strategies the three programs used to obtain financial support.
