“The purpose of this program is to provide information, education, technical assistance, and peer support to families of children and youth with special health care needs (CYSHCN) and the professionals who serve them. The goal of the Family-to-Family Health Information Centers Program is to promote optimal health for CYSHCN by helping families and health professionals to partner in health care decision-making and facilitating access to cost-effective, quality health care.”

Eligibility: “Eligible applicants for the Family-to-Family Health Information Centers program include any domestic public or private entity. Domestic faith-based and community-based organizations, tribes, and tribal organizations are eligible to apply. Eligibility for this funding opportunity is limited to applicants in Delaware, Hawaii, Maine, Montana, and Nevada.”
