“The Annie E. Casey Foundation invites proposals from localities interested in becoming new Evidence2Success communities. Purpose: To assist state and city leaders to work with low-income communities to invest in proven programs that benefit children’s health and development and lead to improved population outcomes. Evidence2Success combines public health and prevention science to offer a new way of helping children and youth meet critical behavioral, educational, emotional and physical milestones. The framework is composed of five core elements:
(1) Partnerships among public systems, elected officials and communities
(2) Strategic use of local data to identify needs and develop a consensus among partners on outcomes
(3) Financing strategies to map current investments, shift funding to cost-effective prevention programs and secure sustainable funding
(4) Capacity building to implement evidence-based programs
(5) Measurement of population-level changes.”

Applications may be submitted by any entity that can secure commitment to the Evidence2Success prerequisites on the proposal cover sheet.”
